Deciding your goals for your care management plan
Would you like someone at the meeting?
You can have another person such as an existing carer or family member with you to help develop your care plan. If you don’t have a carer or a family member who can help, an advocate can be made available through Older Persons Advocacy network (OPAN) by calling 1800 700 600. This is a free service.
Deciding your goals
A goal could be something like maintaining a healthy lifestyle or being able to get around independently. This will help guide the choices you make about which care and services best support your needs.
Some questions that may be helpful are:

- What is important and what goals do you have?
- What concerns do your family constantly voice to you about you living at home?
- What sort of things might help to improve your day to day life?
- What do you look forward to the most in your week?
- What makes your life enjoyable and meaningful?
- Do you feel lonely or isolated living at home?
- What support do you need to stay safe at home?
- At what moments do you feel uneasy about doing something at home?
- Where and when do you want support?
- What specific tasks do you have either trouble performing or tasks that really exhaust you?
- What tasks are you currently performing that you are not confident in? This could be as varied as making it into the garden or taking medicine.
- How would you like to receive your monthly Home Care Package statement? You can receive a paper, email or web-based version of the statement.
